Conway High School Students who are age 16 or older must have a second dose of the meningococcal vaccine (MCV4) if they received their first dose before age 16. Appointments can be made at your health care provider, the Faulkner County Health Department (501-450-4941), the Wampus Cat Care Clinic (limited hours) (501-538-3710), and most pharmacies.

Please send the updated immunization record to Conway High School, fax 501-450-4884, Attention; School Nurse.

Immunization records may also be emailed to Amy Kordsmeier, RN at kordsa@conwayschools.net.

Immunization exemptions may be obtained from the Arkansas Department Health Immunization Division, 501-661-2169, and it is also available online.
